We invented the "referral system"  for elderly care in Oregon in the early 1980's with an emphasis on locating facilities dedicated to quality care, especially those who dispense lots of TLC.

With senior care, different types of facilities have been developing and we have made it our business to know who is providing the best services for what particular needs in what areas.  Some of the choices are Adult Foster Care, Assisted Living, Dementia Facilities or Nursing Homes.

HOW EZ TRANSITIONS OPERATES

At the time you need a facility, you call us and here is what you can expect.  We will conduct a brief interview on the telephone.  We will need the following information to successfully assist you.

1.  Physical & Mental Condition
2.  Age & Social Needs
3.  Geographic Preference
4.  Financial Considerations.
 
We will discuss the alternatives available and then we will match these needs with known  resources in our large, highly selective database.  We discuss the care needs with potential caregivers and call you back promptly with the best choices available.  That's all you need to do! 
 
You make your choice of these pre-screened facilities and there is no obligation.  Bear in mind, that our 26 years of experience will provide only top-rate facilities for you.  That should give you considerable peace of mind.  (If transportation is needed, we can arrange it for you)

Respecting that each resident is unique, we take great pains to locate services tailored to each individual's needs. Our aim is to see that each senior's health, independence, dignity, and quality of life is maintained.

Assisted care residences are designed for comfort and safety. You can enjoy the comforts of home while having the safety, security, and assistance of a professional and caring staff. We know what facilities are available, their varying levels of service and special skills. Most facilities specifically tailor their programs to suit the special needs of their residents.

We are very proud of our reputation for having a commitment to quality care. We will refer you to the kind of facilities you need that have previously demonstrated a similar commitment.
